Diagnostic sample submission

VLA offers diagnostic testing, advice and support to veterinary surgeons in practice for the diagnosis, control and prevention of disease in farm animals.

The information generated is an important part of the disease surveillance carried out by the VLA under contract to Defra. It enables Defra to monitor the emergence of new diseases and changes in the pattern of existing ones so that appropriate intervention to control disease can be made.

In order to maximise the surveillance value of this service, for which Defra provides a significant level of financial support, the VLA requires every submission to be accompanied by a fully completed VLA submission form.

Forms

For your convenience the forms are available below as pdf documents. If you would like a supply of the paper copies please please contact your local VLA Regional Laboratory.

Different forms are required for import or export testing. See the imports and exports page for further details.
