Please only fill out the fields you don't want to be contacted on, e.g if you don't want to receive faxes but would like to receive email, just fill out the fax number field.
If you have a county/parish/holding (CPH) number we use these records to contact you and you cannot be removed from this system. Please keep your records up to date by contacting your local Animal Health Divisional Office.
