Instructions to download and use the 'read only' Adobe Acrobat PDF version of the poultry registration form
To use this mechanism you will need Adobe Acrobat Reader TM version 5.1 or later installed on your PC. This can be downloaded for free from the Adobe website. Minimum PC system requirements are detailed on the Adobe website.Once you have Adobe Acrobat Reader TM installed on your PC, click the link to the English or Welsh registration form on the Poultry Register homepage. The form will open and you should then save it on your computer and print a copy. Please enter all your details on the printed form and send it back to us at the address on the Poultry Register homepage.
